I think our expectation to generate a 1094-C form with DGE information and another one without DGE information.

Our understanding, however, is that one 1094-C form should be generated per EIN, so in this case a single form with DGE information would be generated, even though if only part of the employees are covered by DGE health insurance offer.

Can you double check if our understanding would address your reporting requirements?
